Are you ready to schedule your first appointment with our team at Third Street Dental? First, we ask that you take a look at this page to find out more about what dental insurance plans we accept and how you can prepare for your appointment. If you have any questions about how to fill out forms or would like to know if we’re in-network with your insurance company, don’t hesitate to reach out to our friendly staff.
Your First Visit
When you first walk into our dental office, you’ll be greeted by one of our friendly dental office staff at our front desk area. We’ll input all of the information that you provided us on your new patient forms into your file and take you back to one of our treatment rooms. You’ll have a 1.5-hour appointment with our hygienist and dentist, where we’ll take a complete set of X-rays, conduct a thorough checkup, including an oral cancer screening, and finally, polish and scale your teeth.
New Patient Forms
After you call to schedule your appointment, we ask that you click on the link below to download and fill out our patient forms. Once you’ve completed them, you can either print them out and bring them with you to your first appointment or email them directly to us. That way, you won’t need to spend an additional 10 to 15 minutes filling them out in our waiting room when you arrive.
